Google free credits.

I have submitted the form but didn't got code for credits.
If I submit a new form, will I get credits?
What was deadline for submitting the form and redeeming the credits?

  It states that it will take 5 business days to receive the voucher code, and that the Google credits must be redeemed "by December 14" and then used within 14 (calendar) days of being redeemed.
